1.There are only three reasons that trigger a compulsory cancellation of the student account before the 12th month:

a.      Change of address, being that the new address must be more then 25 miles from the academy

b.      Medical condition, being a permanent disability that will not allow the practice of BJJ for a considerable time (at least two years)

c.      Deployment in case the student is a military

2.Documents to be provided:

a.      Change of address: Cancellation form and new electrical bill

b.      Medical condition: Cancellation form with doctor letter proving the permanent disability

c.    Military Deployment: Cancellation form and document proving deployment

1.There are three reasons that allow a student to freeze their membership                                     (3 month Maximum on all account freezes)

a.    Medical Condition, being that the condition will not allow the practice of BJJ for at least 1 full month

b.    Temporary leave, being that the student will be out of town for at least 1 full month.

c.    Unexpected unemployment

2.Documents to be provided:

a.    Medical Condition: Account Freeze form with doctor letter proving            

the disability

b.    Temporary leave: Account Freeze form and receipts proving leave. 

i.e. plain ticket, hotel receipts

          c.   Unexpected unemployment: Account Freeze form and letter

#                                                 STANDARDS AND GUIDELINES

1

All cancellations and account freezes must be based on the cancellation policy

2

Students may cancel their membership at any time by paying a $210 cancellation fee

3

Students have the option of transferring their remaining balance to a friend or family member. It means that the payments will continue but the account will be changed to the new person

4

All membership agreements are set to auto renewal. If a student chooses not to renew their membership they need to fill out a “DO NOT RENEWEL” form and hand it to the program director at least 2 weeks before the membership self renews

5

All Cancellations and Account Freezes must be done in the academy office. No Emails, Voicemails, etc.

6.

Cancellations and Account Freezes take 7-10 business days to process
